OPEN A FILE
Please follow these four steps to open a new file with Medisys:
- Execute our Fee Schedule Contract (Click). This contract will answer questions you may have regarding our fee structure, travel, testimony, etc. Please indicate requested services, sign, and date.
- Complete the Patient information form (Click), indicating important details about the case.
- Send all relevant medical, education, and vocational records to us in electronic (PDF) form. Please include an index of what is being sent. Records can be placed on a disc/drive, or in a secure online file-sharing program.
- Please send the completed fee schedule contract, patient information form, and appropriate retainer (see Fee Contract) to our mailing address. MediSys does not accept credit cards.

Leigh Anne Levy, RN, MSN, CLCP, CEN
Leigh Anne Levy is the Clinical Director and Owner of MediSys Rehabilitation, Inc., where she has been involved in Life Care Planning since 2008. She received a Master of Science in Nursing from the University of Texas at Austin, is a Registered Nurse, and holds certifications in Life Care Planning and Emergency Nursing. Ms. Levy has been active in the health care field for over 25 years, focusing her work on emergent care and rehabilitation, with an emphasis on catastrophic injuries and chronic conditions. She is the author of several publications and has lectured extensively at local and regional conferences. In addition to her duties and responsibilities at MediSys, she serves as an Adjunct Associate Professor in the nursing program at Austin Community College and is a reviewer for the Journal of Emergency Nursing.

Eduardo Elizondo, M.D., CLCP
Eduardo Elizondo serves as Medical Director for MediSys Rehabilitation, Inc. Dr. Elizondo is a board certified specialist in Physical Medicine and Rehabilitation and Electrodiagnostic Medicine. He received his medical degree from the University of Texas Health Science Center in San Antonio and completed his residency in PM&R at Louisiana State University Medical Center in New Orleans. In addition to being co-chief resident, he served as a clinical instructor in the training program. He is a member of the American College of Sports Medicine and a fellow of the Physiatric Association of Spine, Sports and Occupational Medicine. His training and expertise in PM&R have led Dr. Elizondo to develop care plans for patients who have suffered catastrophic injuries such as spinal cord, brain injuries, and other maladies. He is a Certified Life Care Planner and has been a consultant with MediSys for over 15 years. He has over 20 years of experience in his chosen specialty.

Melissa Lynott, RN, BSN, CLNC
Melissa Lynott is the Director of Case Management Services at MediSys Rehabilitation, Inc. She obtained her Bachelor of Science in Nursing from West Virginia Wesleyan College in 1990. She obtained certification in Legal Nurse Consulting in 2006, and a certificate of completion in Life Care Planning in 2009. Ms. Lynott’s background as a Registered Nurse includes 10 years as an emergency department nurse, and 15 years as an operating room nurse specializing in neurosurgery and orthopedic surgery. She has worked at MediSys since 2008, where her duties and responsibilities have included review and analysis of catastrophic injuries, subject matter research, and internal case management.
